    Everyone is missing one of the biggest clues that he got cured, the final scene when he asked “is it was better to live as a monster or die a good man”,, he looked right at the fire in mark ruffalos hand as he lit the cig, instead of covering the top or avoiding looking at it, and knew the orderlies were coming for him and with no hesitation just walked up to them.

          You forgot to mention that the storm was a complete fiction/hallucination. Cawley points to it being a fiction during his breakthrough with Andrew in the lighthouse. He says something of the sort "I've heard it a hundred times, your partner Chuck, the conspiracy, Rachel, *the storm* ". There's a storm in his fiction because of his fear of water. We have proof that the storm was fictitious in the last scene: look around the asylum's grounds. Not one leaf anywhere. No sign that it had rained, stormed, no branches, NOT ONE. And this is presumably only 48 hours after a storm that characters were comparing to a hurricane. It's also completely sunny outside. Not one time was the sun shining brightly in this film until now. The storm didn't happen.

                            "Doctor" Freeman, the guy who traveled in his "lobotomobile" was absolutely not a doctor. He was friends with a doctor. Once the actual doctor found out what Freeman was doing, using an ice pick, he completely dropped him. Freeman was an honorary doctor. Won a Nobel Prize for killing so many people. JK Kennedy's sister was lobotomized, foj....being a rebellious daughter. She was institutionalized and basically had the mind of a two year old.

                                  This movie is very well written. The end of the story could be interpreted in two ways
                                  1: Andrew is mentally insane. He killed his wife and had a mental break. He could not except reality and thus had to remain on the island indefinitely.
                                  2: Teddy never killed his wife. Teddy was never mentally insane. Everything that Delores said in the cave is true. Teddy gets his mind wiped in the lighthouse and lives the rest of his days out on the island, trapped there by the doctors so that he would not tell the world the truth.

                                  I personally believe that Andrew truly is insane. But the amazing way this story is written makes it so that you can look at it two different ways.

                                      HES NOT CRAZY

                                      Here's my argument :

                                      The Boat: How the fuck did Teddy get on the boat if this is all an elaborate scheme to bring him back?

                                      The use of water: Every scene with water makes him ill, or some revelation about how the Asylum is evil comes to play, when he's in the cells talking to George, his wife is wet and there's a significant amount of blue in the scene, he tells Teddy "Don't listen to her" and "This is all for you" telling him that his illusions are results of the drugs he's being given, at they are not real, neither is their lies. Remember the woman who tells Teddy to run, asks for water and DOESNT EVEN DRINK IT, signifying that there its all a lie, there is no glass, its all fake.

                                      The fire: Every scene with fire is the truth, the matches, the fire in the cave, his wifes death, George, etc etc, this is because his wife did die in the fire and not the water.

                                      Ruffalo and the drugs: Pretty self explanatory, Ruffalo isn't a detective, he is the Asylums doctor and is trying to rope Teddy in, think about the cigarette at the edge of the cliff, pretty coincidental, the jacket in the water, they are trying to make Teddy mad by pretending he had no partner, that he was the doctor all along,hence the apparent cliff death.

                                      His kids: Teddy had no kids, the kids we see, the fake Rachel with the black hair, were from the concentration camp he saw when he was a soldier, they were a dead family, and the asylum used that to pretend he had a family, remember what Real Rachel said in the cave, they try to find a past incident of trauma in your life and use it against you. He also says at the end he had no kids.

                                      At the end: Teddy does not seem Crazy AT ALL, infact he seems completely normal, kind of strange considering he's some wacky lunatic that killed his wife. No, he's aware he's never leaving the island, he can't escape, so he fools everyone in thinking he's hopeless, by telling them he's Andrew, then he's Teddy again, so he becomes Lobotimized and rids himself of the nightmare. Live a monster speech. Last scene is of the Lighthouse after they have taken him away. Because that IS where they do experiments on people. Think about it, ITS THE LAST SCENE of the movie, if he IS crazy why not end it sooner? Why is it ambiguous if it's common knowledge?

                                      Inconsistencies with their so called "Role play" : This whole so called scheme to make him realise what he has done relies on ridiculous principals, trusting a lunatic to roam free around the grounds with past violent tendancies as they say, interviewing other patients, when they would surely give up his identity to him, if he was Andrew, they are mental, how could you trust them not to? Withdrawal from his meds? BULLSHIT, that's such a lie, he had the migraines because they were giving him the drugs, the shaking too, pill withdrawal does not make you shake like that. Him on the boat? He could jump off and escape easily if he suddenly snapped out of it and realised who he was. No, no no no it makes no sense this plan, they don't do it for any of the other patients but him? HUH? Why? That makes no sense.

                                      Teddy is innocent, they are trying to make him mad so he doesn't blow the lid off their operations (Also Nazis) think about Scorcesse, he is a master of film, it's too easy to just give away the ending with some BS explanations and monologues, think about it.

                                          One thing people noted was the Dachau scene showed snow and frozen bodies. But Dachau was liberated in late April 1945 when there shouldn't have been any snow or cold weather.

                                              Something i noticed now rewatching this video, at 22:34 the doctor shifts on the stairs and his facial expression seems to indicate that something just occured to him, maybe he realised at that point that the treatment stuck but Andrew choose to go through with a lobotomy as to not have to live with what he did

                                                  the writers not saying whether or not the therapy worked is so fucking stupid leaving a movie completely without a true ending is something that rubs me the wrong way its such a cheap ass lazy way to end a story hahahahaha we will leave it on a cliff hangar then never say becuase its "up to the watcher to decide" No fucking tell us the ending its such a fucking cop out.

                                                                    I watched this film for the first time today (I know I'm bad/late) and it is amazing. I figured out he was delusional but I didn't figure out the twist, I was mindfucked. 10/10 film, I needed a good watch after the letdown that was IT chapter 2. I'm gonna have this on my mind for a while now.

                                                                                Mrs. Kearns writes "run" on the paper she slips to Teddy because she knows he has an opportunity to escape while they're doing the whole role play experiment. It's also why she sounds "coached" about what to tell Teddy - she has been. As for the conspiracy about why Mrs. Kearns' hand is empty when she goes to drink the water Chuck brings...she's crazy, let's not forget that. I doubt it was an error in editing.

                                                                                The creepy lady in the yard at the beginning of the film does the "shush" motion at Teddy because she knows him, knows that she's playing a game and has been instructed not to spoil it. She's a crazy lady enjoying a game, that's all.

                                                                                Pay attention to the staff interview scene. When Teddy and Chuck interview the nurses and orderlies it's easy to see just how ridiculous the staff finds the interview. One nurse says something about how 'far from normal' their jobs are - she's making an ironic joke because she's talking to a lunatic dressed as a cop. In that scene, the staff are also not too enthused about the role play, and Dr. Sheehan / Chuck pushes them to answer Teddy's questions. You'll see what I mean.

                                                                                When Teddy reveals to Dr. Naehring that he's figured out the patient 67 riddle during a staff meeting, Naehring says "What are they doing here?" He's genuinely annoyed that Dr. Cawley is letting Teddy/Andrew roam so freely.

                                                                                In Ward C Teddy is accosted by a loose prisoner and nearly strangles that prisoner to death. "Chuck" and a guard show up and drag the strangled man away. The guard tells Teddy that he can't come along to the infirmary, while mumbling about how much trouble he's going to be in - for letting a patient strangle another patient.

                                                                                Watch the guards throughout the film. They get extremely edgy whenever "Teddy" is around, and clutch their guns a little tighter. This is especially true at the beginning when "The Marshals" come to the island. It's because the guards know Teddy is a lunatic and they're not exactly thrilled about the role play experiment. It's also why they are less than enthused about looking for a Rachel Solando who doesn't exist down by the ocean rocks.
